Package sessions are purchased in a single transaction prior to scheduling the first session in the package. All session must be used by expiration dates as follows: 5 session packages must be used within 8 weeks from first day of service. 10 Session packages must be used within 13 weeks of first date of service. If not used in the stated time frame, sessions are forfeited. No refunds will be provided in any circumstances for package purchases. Please read and sign financial agreement prior to f
